SAMPLE LETTER TO EMPLOYEE RE: PERSONAL LEAVE OF ABSENCE Dear [employee name]: We have received your request for a personal leave of absence for the period from [dates of leave]. The firm will approve this request in accordance with firm policy including: Your personal leave of absence will be granted up to 30 days in a calendar year for business-related, educational pursuits or personal reasons. When all available vacation time has been exhausted, personal leaves of absence are without pay. You will be responsible for the full cost of insurance benefits while on leave. During the personal leave of absence, you will not accrue any [PTO, vacation or sick leave] and will not be entitled to holiday pay. We request that you notify the firm at least one week in advance of your intended date of return. When you return to work, every effort will be made to re-instate you into the same or substantially similar position as the position you held before leave. However, if business circumstances prevent the firm from offering you the same position upon your return to work, an alternative position and corresponding salary will be offered if one suitable to your skill and experience level is available. If you fail to accept the offered position, if a change is necessitated, this will be deemed a voluntary termination of your employment effective immediately. Further if you fail to return from your leave of absence on schedule or fail to report to work in a timely manner, you will be considered to have voluntarily terminated your employment, effective at the close of business of unreported absence. _____________________________ Firm _____________________________ Employee
