The team that gives it

Nurses

Hospice Care Network nurses ease not only our patients' pain, but their loved ones' minds. They care for the patient's physical needs, visiting as often as necessary to relieve their pain and the symptoms of their illness. But their care goes far beyond medicine alone - our nurses encourage, reassure and inspire. They provide the support that helps families get through some of the most difficult times in their lives, instilling the confidence that turns mothers, fathers, sons and daughters into expert caregivers.

Physicians

The physicians of Hospice Care Network's team of medical directors all specialize in one thing: compassion. Our medical directors are experts in oncology, hematology, internal medicine, family medicine and pediatric care, combining their special knowledge of our patients' illnesses with the belief that people have the right to live their remaining time in peace and dignity. They review each and every patient's medical needs and prescribe plans of

Hospice Care Network's home health aides offer caregivers an extra pair of hands and a shoulder to lean on. Our home health aides give the hands-on care that keeps patients comfortable and relieves families of some of the responsibilities of caregiving, so they can enjoy their time together. Whether it is helping with bathing or cooking a meal, Hospice Care Network's home health aides support our families and make their lives a little bit easier. We believe in the importance of keeping our patients' and families' day-to-day lives as uncomplicated as possible that's why we employ one of the largest staffs of home health aides of any hospice in our area.

Home Health Aides

Chaplains

In the final stages of life there are often questions of spirituality. Our chaplains help patients and families find their peace. The chaplains of Hospice Care Network's Pastoral Care Team come from diverse religious traditions, working together to satisfy the spiritual needs of patients and families with respect for the values of all faiths. They provide care for the soul, whether the patient be Christian or Jewish, Hindu or Buddhist, atheist or agnostic. This spiritual care is often what gets families through this most difficult time in their lives.

Volunteers Social Workers

Volunteers logged 17,750 hours in 2003 - that's 17,750 hours of dedication, care and compassion, given from the heart. Patients and families often need a friend, a companion to help them get through the difficult hours. Hospice Care Network's patient care volunteers serve as that caring presence, that someone who is there simply to hold a patient's hand or run an errand for a busy caregiver. Our administrative volunteers are equally important to our organization, providing a number of valuable services behind-the-scenes in our offices. HCN's Board of Trustees members are also volunteers, generously giving their time to lead our organization. In ways big and small, hospice volunteers give the most priceless gift of all: themselves.

There are many emotional issues we face when dealing with death and dying, whether it is our own life or someone we love's. Hospice Care Network's social workers shepherd our patients and families through this time of sadness and uncertainty. Because each family has its own circumstances, one size does not fit all when it comes to hospice care. Our social workers immerse themselves in our patients' and families' lives, learning their unique social and emotional needs and working to satisfy them. They may provide emotional counseling or identify community resources that can offer extra assistance - whatever it takes to make the hospice experience go as smoothly as possible. Tailoring hospice to the needs of each family that's the mission of our social work team.

Administration

It takes a lot of people behind the scenes to make a healthcare organization run like a well-oiled machine. Each member of Hospice Care Network's administrative staff plays a vital role in making our patient care the best it can be, whether they serve as an accountant, fundraiser or human resources representative. They are not only very good at what they do, they are also keenly aware of just how important their job responsibilities are to the care our patients and families receive. At Hospice Care Network, all of our staff members are in the business of patient care.

Bereavement Counselors

Sorrow may come with the territory when it comes to hospice, but Hospice Care Network's bereavement counselors focus on finding the joy as well. Our bereavement counselors guide patients, their families and the community at large through the grieving process, serving as both confidants and compassionate advisors. This team of professionals can skillfully engage a child who has lost someone important to them, comfort a person who has lost a spouse after years of marriage, and inspire renewed hope in a parent who has experienced the loss of a child. Hospice care is just as much for the living as it is for the dying - our bereavement counselors are there every step of the way

A

t Hospice Care Network, we are in the business of caring. While for-profit organizations report on their sales of the product they put into the marketplace, our "product" is more difficult to quantify. Our product is the comfort a patient feels being able to live their remaining days in their own home, with those they love around them. It is how we tell caregivers they're doing a wonderful job in taking care of their loved one. It is holding the hands of those who have lost someone important to them, and helping them find the strength to go on. At Hospice Care Network, our team is comprised of professionals trained in many disciplines of care, each an essential and inextricable piece of our patients'and families' experience. Each discipline may constitute "care" on its own, but not until they all come together can they be considered hospice care. Our professionals truly work together seamlessly to share their talents and special understanding of what patients and families need at the end of life. In 2003, we had the opportunity to expand our concept of care further than the traditional hospice model with the launch of programs within the Marks Center for Caregivers, including the Care Connection, a community resource center managed by a dedicated social worker, and van transportation for families of patients in inpatient hospice care. We also laid the final groundwork for The Hospice Inn, our freestanding hospice center, which opened this February to rave reviews from patient/families and the medical community alike. We could not have made such strides in the care we provide without our wonderful team, which manages to look to the future while at the same time being so very much in the present when it comes to caring for our patients. his past year at Hospice Care Network has been one full of excitement and anticipation for great things. It was a year where some of the dreams that we've had for this organization have been realized, and others have begun to take flight. Anyone involved in launching a new care facility, as we did this year with the opening of the Hospice Inn, knows how much energy, drive and hard work go into such an endeavor. Two thousand three was a year of working with architects and engineers to design the space to meet our patients' needs, gaining approvals from governing bodies, and forming a wonderful new team designated to serve this facility. Our stellar Board of Trustees members and dedicated staff and volunteers worked tirelessly to make the Hospice Inn all that we envisioned it to be: a top-notch medical facility that feels more like home than a hospital. We also got off to a strong start with plans for our freestanding hospice unit which will be built on the Manhasset campus of North Shore University Hospital. We are moving full steam ahead with this project, and are grateful to all who are making this dream possible, including our generous donors and the North Shore-Long Island Jewish Health System. As chairman of the Board of Trustees of Hospice Care Network, it is personally fulfilling for me to see our organization flourishing. From time to time I reflect on our humble beginnings and marvel at the road we've traveled to become such a world-class hospice organization. But even as we've doubled, tripled and quadrupled our patient census and our staff and volunteer numbers have grown exponentially, we still retain our down-to-earth values and commitment to providing the best care possible to our patients and their families. Hospice Care Network President and CEO Maureen Hinkelman became Board Chair of the National Hospice and Palliative Care Organization, the oldest and largest not-for-profit organization actively involved in advancing care for those at the end of life.

HCN launched the Marks Center for Caregivers, a first-of-itskind caregiver support program made possible by a major gift from Board of Trustees Member Cynthia Marks and her husband George.

Bernard G. Brophy, III was honored by Hospice Care Network at the Friends of Children's Bereavement Annual Dinner-Dance fundraiser at Captain Bill's Bayview House. This always enjoyable event raised $71,000 to support the children's programs of Hospice Care Network.

Karen and Frank Boulton were honored for their dedication to the hospice mission at Hospice Care Network's Annual Cocktail Party, held this year at Barbara and John Weir's Home in East Islip. Two hundred and twenty hospice supporters attended this event, raising $50,000 for the programs of Hospice Care Network.

Friends of Children's Bereavement

Hospice Care Network held its Annual Board Meeting at the Unitarian Universalist Congregation at Shelter Rock, honoring Board members and employees for five, ten and fifteen years of service.

Teeing off for a good cause, 170 golfers raised $99,000 at Hospice Care Network's Annual Golf Outing, held at the Sands Point Golf Club and the Village Club of Sands Point. News 12 Long Island featured HCN patient care volunteer Stanley Friedlander as part of their "Making a Difference" program. J. King's Annual Golf Outing held at the Timber Point Country Club raised $50,000 to benefit Hospice Care Network.

Enthusiastic friends of Hospice Care Network took to the boardwalk at Jones Beach State Park for the organization's Annual Walk-AThon, which raised a net income of $15,000 to benefit HCN's children's programs.

Seventy-seven children attended Hospice Care Network's Eighth Annual Children's Bereavement Camp, held at the Coleman Country Day Camp. With a theme of "Be a Survivor," kids participated in musical and adventure-oriented activities designed to foster their strength, courage and hope. Stadtmuller Jewelers of Sayville hosted its Annual Holiday Shopping Benefit with raffle proceeds going to Hospice Care Network. Hospice Care Network received a generous $12,000 contribution from the Long Island Charities Foundation. Clinical Outreach Associate Noreen Sokota, RN was named HCN's Woman of Medical Achievement, an award given by the North Shore-LIJ Health System celebrating women in healthcare. Noreen was nominated by her peers for embodying the values the System advocates ­ excellence, caring, customer focus, team work, integrity and innovation.

Hospice Care Network registered nurse Carol Fox was selected from more than 400 canidates as a runner-up in the New York Times Job Market Nursing Awards in recognition of her dedication to patient care.

In 2003, HCN received over 10,000 gifts from individual donors, businesses, corporations and bequests. Whether a memorial, a grant to support a specific program or participation in a special event, each gift is important and appreciated. A gift may be a few dollars from a family to many thousands from a foundation; each one makes a significant difference in helping HCN provide important services to our patients and families.
