Registering Your HP ITSM/ITIL, PM and BA Attendance with PMI

HP Education and Management Concepts, Inc. are PMI Registered Education Providers.

This document gives guidance on claiming PMI contact hours / PDUs for HP Education's ITIL/ITSM, Project Management, and Business Analysis courses. Note: If you are already PMP certified, you can claim PDUs online at http://www.pmi.org. Use the information below in steps 5 and 6 to complete your online submission accurately. Claiming Contact Hours to Accompany Your PMP Application If you are preparing your PMP application, your submission will need to include one or more Activity Report Forms (explained below) identifying at least 35 contact hours required for application. 1. If you have not already done so, you must become a PMI member and obtain a PMI ID#. You can register for PMI membership by going to http://www.pmi.org and clicking on the "Register Now" or "Join PMI" link. 2. If you have not already done so, review PMI's Continuing Certification Requirements (CCR) Program Handbook (http://www.pmi.org/CareerDevelopment/Pages/Continuing-Certification-Requirements-(CCR).aspx). 3. From the above handbook, print one copy of the Activity Reporting Form for each course for which you are claiming contact hours. 4. Search the course list on pages 2-3 to find the course(s) you took. Note that for the purposes of your PMP application, only Project Management courses are accepted. 5. For each course, identify the Course Title, Course Provider, and the number of contact hours / PDUs. 6. Complete the form, making sure to use the exact values that follow for the specified fields: a. If the course is Category 3: i. Provider ID: use the provider ID specified for the course ii. Program Number: the course number shown on this list b. Program/Activity Title or Formal Name of Course: the exact title shown on this course list c. If the course provider specified in the course list for this course is "HP", then i. Name of Activity Provider: Hewlett Packard (HP) ii. Provider Address: 3000 Hanover Street iii. Country Code: 01 iv. Area/State/City Code: 800 v. Provider Phone Number: 4725277 vi. Provider Email: [email protected] d. If the course provider specified in the course list for this course is "MC", then i. Name of Activity Provider: Management Concepts, Inc. ii. Provider Address: 8230 Leesburg Pike, Suite 800 iii. Country Code: 01 iv. Area/State/City Code: 703 v. Provider Phone Number: 2704060 vi. Provider Email: [email protected] 7. Submit the completed form(s) as directed at the bottom of the form.
