We are a small Surgery offering personal primary healthcare in a calm and comfortable environment. We aim to provide a high standard of medical care in a friendly and professional manner. You can help us achieve our aim by reading this guide and following the suggestions to ensure that appropriate services can be provided when you most need them.

Violent or abusive patients

The Surgery has a zero tolerance policy on violent or abusive patients. If any of our patients or prospective patients are rude, abusive or violent to any practice staff they will be removed from, or refused to join, our list.


There are spaces at the back of the Surgery for patients and staff. We are on the corner of Eldon Road and St Johns Road: however there is no vehicular access to St Johns Road from Eldon Road ­ you should drive up St Johns Street and then along St Johns Road and the surgery is at the end on the left.

Private Fees

You may be charged for non-NHS work undertaken. This includes Private Medicals, Insurance Forms, some Travel Immunisations, signing of Passport photos etc. Medicals are not carried out during normal consultations; please state when booking an appointment that it is for a medical.

Please attend or telephone the Surgery any time during working hours to make an appointment to see the doctor, nurse or other member of our clinical team. Consultations usually last for 10 minutes with the Doctor; times vary with the Nurse and the other clinics depending on the need. If you wish a friend or relative to accompany you they may do so except for Counselling. Please think twice before making an appointment ­ do you really need to see a doctor? · · · · Have you tried simple home treatments? Our Health Visitors (957 4844) can assist with many problems related to babies and post-natal mums Your local pharmacist can provide treatment for many common conditions and advice about staying healthy, choosing medicines for common ailments as well as advise if you are unsure about seeing a doctor. The NHS Direct service offers general health advice, answers general health questions and is available on 0845 4647. All this and self-help can be accessed online at .

Urgent Appointments: If you require an appointment for a problem that you feel will not wait until the next available routine appointment, you will be given a time to attend usually at the end of morning surgery or at the beginning of afternoon surgery. You may not be able to see the doctor of your choice and may also have to wait longer than usual. Telephone Appointments: For clinical issues that can be resolved on the phone the Surgery offers a limited number of slots at the end of morning surgery when you can speak to the Doctor or Practice Nurse. These appointments should be booked on the day via Reception. Please be prompt and please cancel your appointment if you can no longer keep it or no longer require it; someone else may need to see the doctor. Saturday Appointments: we now offer a limited number of appointments on Saturday mornings. These are for routine consultations only and must be pre-booked. For urgent or emergency issues on Saturdays see "Out of Hours" below. Home Visits: can be arranged for those patients too ill to attend the surgery. Please use the service thoughtfully and ring as early as possible to request a visit. The receptionist may ask for further information to enable the doctor to determine the priority and nature of the visit. Out of Hours (Evenings, Weekends and Bank Holidays): The Surgery provides 24 hour cover via WestCall. If you need urgent medical attention you should first call the normal surgery number. Travel vaccinations: For information regarding vaccinations please advise us at least 4 weeks before your planned departure of the places you will be visiting so the nurse can assess your needs based on your immunisation history. At that stage we will be able to advise an appropriate duration for an appointment to agree the way forward. Please note that not all travel vaccinations are free of charge.

For patients requiring regular repeat medication it may be cheaper to apply for a prescription prepayment card ­ for details please ask at Reception. Repeat Prescriptions: If you need a further supply of your medicines please hand in the white repeat slip with the items you need clearly marked or write down what you want and hand it in. Please note that if your white slip states you need to see the doctor then reception are unable to process your request ­ you really do need an appointment with the doctor! Prescriptions are ready for collection within 2 working days after your request is received by reception. Repeat Dispensing: offers an alternative way for patients with long term conditions to obtain repeat medication at their local pharmacy without the need to contact the Surgery for each prescription. Please verify your local pharmacy participates in this scheme. Electronic Prescription Service: Eldon Road Surgery plans to implement the paperless version of this service in 2008, if the centralised facilities allow.

Health checks

Patients aged 75 or over who have not had a consultation in the past year and adult patients under 75 who have not had a consultation in the past three years all have the right to request a health check consultation.

Sickness Certificates

A self-certification form (SC1) can be used for the first week of absence due to sickness without consulting the doctor. This form is available from your employer or the DSS. The doctor can provide a doctor's sick note for patients they have seen and who are off work for more than a week.

Patient Confidentiality

Information provided is treated with the strictest confidence by staff employed at Eldon Road Surgery, in accordance with relevant legislation.

Right to see your records

If you wish to see your medical records held at Eldon Road Surgery, this can be arranged. A copy of your computer record may be obtained under the Data Protection Act for £10. Please speak to Reception or the Practice Manager for further details.


We endeavour to provide a high quality healthcare service at all times ­ if you feel you need to complain about your treatment as a patient, please speak to or write to the Practice Manager. Please put any other comments (good or bad) you may have in writing to the Practice Manager.
