Chapter Divider EXAMPLE!

Title--related to the theme of the portfolio (sports) AND the topic of the chapter (family)

Artwork: A drawing, a picture, a photograph, clip art or something you create with scrap booking supplies

Name the topic of the chapter (Family and Me)

Name:____________________________________

Chapter titles should connect to both the THEME of the portfolio and the content of the particular chapter. Physical Me is full of information about you as a person. Your chapter title should have something to do with you as an individual and relate to your theme. Fiction & Me includes fictional stories you wrote in 8th grade. Names of stories related to your theme, imaginary, nearlyimpossible or ideal situations also work. America & Me looks at how you fit into your community. Your chapter title should have something to do with the broader setting that your theme belongs to. Math & Me includes projects completed in math class. Chapter titles should incorporate ideas related to math: time, measurement, money, numbers, etc. Identity & Me is full of projects that show off your family's own history and how you fit into that. Choose a chapter title that refers to things or events in your theme's past or how long it's been around.

Choice Chapters (2 required) can be chosen from a variety of topics. Pick titles that match your theme and your selection. Write at least 3 paragraphs for each chapter. You may also use projects that are completed in other classes, but you must see me for approval. Creative Me My Friends & Me A Phobia & Me Me Back When... Laugh The Future Me My Family & Me Nature & Me Sports & Me The Relaxing Me Washington D.C. & Me The Private Me My Pet & Me Habits & Me What Makes Me Music & Me

Dedication page You must dedicate your portfolio to a significant person in your life. Choose one or two people and give specific reasons to explain why you chose them. ("He's always there for me," IS NOT a specific reason). Your dedication should be at least 1 paragraph. What I Learned About Myself This should be at least 2 paragraphs in length. Reflect back on the events of the past year. What have you learned about yourself, your skills and talents (or lack of!), your interests, how you relate to others, etc.? Did you end up with a class you didn't request and find out you enjoyed it? Did you have to take on new responsibilities at home? Did you try a new sport, meet new people or lose a friend? What did you learn from these events? Be specific. Prologue (1 paragraph) & Epilogue (2 paragraphs) The prologue is the introduction to your theme and portfolio and the epilogue is the conclusion. Be creative--start with a quote or an anecdote; end with a descriptive passage. Both should show how your theme is an important aspect of your life. The epilogue can give the reader a taste of the future you see for yourself.
