FAQ on Humana's New Vision Programs with EyeMed Vision Care, Inc.

Q What do I do if my doctor /the EyeMed provider doesn't give me the discount rate Humana stated I should get? Call EyeMed customer service at the toll free number dedicated to Humana members, 866-392-6056 for discount only, 888-289-0595 for those with an insured plan. It will be helpful if you have a copy of your bill from the provider in front of you. Their Customer Service representatives will be glad to review your service with you. If they can determine that you indeed did not get the promised discount, they will contact the provider on your behalf and see that the error is corrected. What if my doctor isn't in the EyeMed network? How do I get him/her added so I can get the discount? EyeMed will be happy to review any nomination requests to solicit the doctor for participation in the EyeMed network. You can go to MyHumana.com and access a nomination form from there. Please note: in can take up to 90 days to complete the credentialing and approval process before a provider can be added to the EyeMed panel, and EyeMed cannot guarantee that all providers will accept the invitation to join the network.

Do I have to go to LensCrafters to get my glasses made to get the discount? Can my private EyeMed provider do both my exam and sell me my glasses? You are entitled to the discount from any EyeMed provider you choose. All private EyeMed providers are able to sell you glasses as well as perform your exam. By your own choice, though, you can have your exam from one provider and buy your glasses from another provider. As long as the provider is part of the EyeMed network, you will be entitled to your discount/insured benefit.

I've seen other groups (such as AAA) offer a LensCrafters discount of 30% off the exam at the independent doctors located next to the LensCrafters. Humana's eye exam discount isn't that good. Why? Humana's discount program offers greater overall savings than these programs that offer a discount on the exam only.

Is there a limit to the number of times per year that I can use my discount? No, you may use the discount to purchase new eyewear as many times as you wish.

I'm insured by Humana/am a Humana associate, but my child/spouse is not. My child/spouse needs glasses. Will my discount apply for them? Yes, the discount includes all Humana insureds/associates and their legal dependents.

What identification do I need to get the discount on services/materials?

You need only show your Humana medical id card, your Humana dental id card, or the Humana discount card that is available from MyHumana.com or the Humana associates benefit center on HOWIE. Can I get my exam at my private doctor, who is not an EyeMed provider and does not want to become one, and then go to a LensCrafters to have my glasses made? Will I receive the discount on these services? Yes, you can have your exam done by your private doctor and then go to LensCrafters to have your glasses made. While you will not receive the discount on your exam since the doctor does not participate with EyeMed, you will get the discount on your materials purchased from EyeMed or from any of the other providers who do participate in the EyeMed network. What should I do if my provider, who is listed in the EyeMed provider finder tells me that I'm not eligible for the discount or that he/she doesn't honor the discount? You should call EyeMed Customer Service at the toll free number dedicated to Humana members, 866-392-6056 for discount only, 888-289-0595 for those with an insured plan. They will be happy to assist you in resolving any issue you have with the program/providers.

Does this discount apply at providers/LensCrafters out of state? I spend the winter months in Florida and don't want to travel back to my home state if I break my glasses. Your discount will be given by any EyeMed provider, nationwide (with the exception of New Hampshire where discount programs are prohibited by state law). I have both Humana medical insurance and Humana dental insurance. Does that mean I get double the discount? No, sorry, but only one discount per service encounter will be given. However, there is no limit on the number of times you or your legal dependents may utilize your discount.

As a Humana member, I currently have a discount for eyewear from Cole Managed Vision. Does this EyeMed discount plan replace the Cole plan? Yes, as of May 1, 2003 EyeMed Vision Care will be the official provider of Humana's vision discount program.

I have an insured benefit with a copayment required for an eye exam. How will this coordinate with the EyeMed discount plan? You will given your exam under your insured plan and must pay the copayment as required by that plan. You can then use the discount plan to purchase your eyewear. The discount plan will not reduce the copayment amount you owe for the exam but will give you great savings on the materials you may need/want to purchase.

I have an insured benefit with an allowance for materials. How will this coordinate with the discount plan?

The provider will apply your insured benefit to your purchases as dictated by your benefit. He will apply the discount plan to purchases not covered by your insured benefit.

Can I go to a provider other than the ones listed in the letter I got to tell me about the discount plan? Yes, the letter listed the ten providers geographically located closest to you for your information and convenience. You may access your discount from any provider in the EyeMed network, nationwide.

If I have an insured vision benefit, do I also have the discount plan? Yes, the discount acts as a wrap around to your insured benefit and will be applied to applicable purchases not covered by your insured benefit.

The letter indicated that costs might be higher in certain states. I live in one of those listed states. How much higher is the cost here? No more than $15 higher, and this applies only to the cost of standard plastic lenses.

The letter says the discount is a percent of U & C on some items. What is U & C? In this case, U & C means the retail price normally charged by the provider who gives you your service/sells you eyewear. The letter says I get a discount on laser surgery. How do I find a laser vision provider? You call 1-877-5LASER6 for the nearest facility and to receive authorization for laser vision services.

I want to buy two pairs of glasses at the same time. Will I get the discount on both pairs? Yes, the discount will be applied to both purchases.

Is there a money-back guarantee on my eyewear purchases? LensCrafters provides a 30-day no risk money back guarantee on eyewear purchased from their stores. Products purchased from independent providers are subject to the warranties given by that provider and/or manufacturer. Provider policies vary by location. Luxottica frames, sold by all EyeMed providers and the parent company of EyeMed, are fully guaranteed for 2 years against any defects in materials and/or workmanship.
