What can the VNA do for you today?

Helpful answers can be a rare commodity in the rapidly changing health care world. Many answers are of little or no help ­ the ones that start with, "Well, maybe..." or "We'll see..." or "You might try...." The more useful answers begin with more reassuring words, "Yes, we can help you with that." At your Visiting Nurse Association, we take pride in giving you reassuring answers. Every day we provide helpful answers to "Yes! We know right where that is. In fact, we are caring for someone near there today." The VNA is of this community and unequivocally committed to this community. Sometimes our answers may not be exactly what you want to hear. For example, you may ask, "Will Mom recover enough to live independently again?" Our answer may be, "That is not likely, but our private care and long-term care services can help to preserve much of her independence." Call us and we will work to allay your fears and concerns, and those of your loved ones. Our customer service standards reinforce what we have strived to do since our founding in 1906 -- to be dependable and professional, courteous and respectful, caring and compassionate, and to address your needs in a timely fashion. From help with care for an aging member of your family to making sure you remain as independent as possible, the VNA is there for you in your time of need. End-of-Life Care Adult Day Program

Our program offers a safe and home-like setting for elderly people, and adults with physical disabilities or other medical needs. Programs are available in Colchester, Williston and South Burlington. Participants may also choose to receive services through a collaborative relationship with the PACE center at the Fanny Allen campus. Adult Day Program services include:

Family and Children's Services

We offer parents through pregnancy and early childhood years the information, nursing care and support necessary to fulfill their capacity as parents and to maximize each child's physical, social and emotional development. Focusing on prevention and early intervention, our services help make a difference for hundreds of children and families each year. Family and Children's services include:

Our VNA End-of-Life Care Services offer comprehensive and coordinated support for those living with a lifethreatening or terminal illness. Our compassionate end-of-life care experts provide skilled nursing care, personal care and homemaker services, and psychosocial, spiritual and emotional support for individuals and their families. Whether it is one's home, a nursing home or in our Vermont Respite House, we provide an array of services to help support patients and their families. End-of-Life Care services include:

Private Care

The VNA Care Connection program offers a variety of personal and medical home health care services from companionship and in-home health care to geriatric care management and private duty nursing services. Individually designed to help people receive the care they need when and where they need it most, the program can help supplement services covered by health insurance policies. Care Connection services include:

Comprehensive Care for Adults

Adult Home Care, the largest program at the VNA, provides a wide range of high-quality care across Chittenden and Grand Isle Counties. Care is individualized according to each client's medical needs. Our team includes nurses, licensed nursing assistants, rehabilitation therapists, social workers, mental health nurses and a registered dietitian. Adult Home Care services include:

Long-Term Care Services

The goal of our long-term care services is to help participants maintain independence in the setting they prefer, to enhance the quality of life for both the client and their caregiver and to acknowledge and value the unique contribution of each family member. Long-Term Care services include:

Nursing care and oversight. Therapeutic activities for mind and body. Healthy meals. Convenient hours. Opportunities to meet new friends in a safe and homelike environment.

Prenatal, postpartum and newborn care. Skilled nursing, Social Work, PT, OT and Speech Therapies. Long-term care for children with chronic medical needs. Family Support Services for parenting education, support and wellness. Parent-Child Center services at the Janet S. Munt Family Room include:

- Playgroups - Parenting education and support groups - English language classes - Infant massage and yoga - Fathers' groups in four communities (FACT) - Nutrition and cooking information for families with young children

Hospice of the Champlain Valley for those in the final stages of life. Palliative Care for people living with a life-threatening or life-limiting illness. Vermont Respite House, a home away from home for people with a terminal illness. Madison-Deane Initiative, a community outreach program providing education and resources. Grief and bereavement programs such as counseling, support groups and Camp Knock Knock, a weekend bereavement camp for families. Pediatric supportive care services for children with lifelimiting or life-threatening illnesses. Volunteer support for patients and families, including the Noyana Singers, our Hospice chorus.

One of the first questions Steve George asked after his hip surgery was "How do I get setup with VNA care when I go home?" Having received VNA care in the past, Steve knew how important nursing and physical therapy visits would be to a speedy recovery. Steve's care team represents the largest program of the VNA, Adult Home Care. Clients needing care following an acute illness or surgery can turn to the VNA home care services for help. Our highly qualified staff of nurses, licensed nursing assistants, rehabilitation therapists and social workers are committed to helping clients recover in the comfort of their own home. Josie Kruse, Steve's nurse case manager, met with him at his home upon his discharge from Fletcher Allen. During her evaluation visit, Josie and Steve discussed services he would need, possible adverse effects of his surgery to watch for and issues around pain and medication management. Steve's PT, Kelly Garaventa, initiated walking activities, created an exercise regimen, tended to his wound dressing changes and evaluated the house to accommodate his wheelchair and walker. She also assessed his progress and made necessary adjustments to his program during her frequent home visits. "I think the VNA is wonderful," says Steve. "I'm able to get the great care I need without leaving my home and that makes me feel better already!"

For 98-year-old diehard Red Sox fan, Irma Isham, listening to radio game broadcasts from the comfy chair in her living room is a pleasure she doesn't take for granted. With little family in the area to help her with daily activities, Irma still lives in her home of the past twenty years with the help of twice-daily VNA caregiver visits. The VNA Care Connection Private Care program provides individualized services available by the hour or round-the-clock. Private Care services help elders and other individuals who need assistance with daily care or other support to remain safely at home. "I hope she can stay as independent as she is for as long as possible," says Terrie Froehlich, one of Irma's evening personal care attendants. "She is an inspirational lady." Providing help with housekeeping, cooking, companionship and transportation for errands and appointments, Irma's caregivers keep this self-sufficient senior healthy and secure in familiar surroundings. "I am grateful that the VNA can help me stay in my own home," says Irma. "I'd hate to be anywhere else."

It isn't safe for my father to be home alone while I'm at work. Does the VNA offer an Adult Day Program?

Adult Day participants may also choose to receive services through a collaborative relationship with the PACE center on the Fanny Allen campus. The Adult Day Program offers opportunities to socialize in a safe and home-like environment with nursing oversight. Activities throughout the day include games and puzzles, cooking, daily exercise, current events discussions and more. For Hal, it's a chance to remain active as he deals with Alzheimer's. For his wife and daughter, it's a break from caregiving and peace of mind that Hal is in a safe and nurturing environment.

Harold (Hal) Stern arrives every morning at the Adult Day Program at Grand Way Commons in South Burlington with the same question, "What do you want me to do?" Hal is 83 years old and has Alzheimer's disease. He attends the Adult Day Program five days a week. The VNA provided care to residents of over 40 different Vermont cities and towns last year. In addition to generous support from individual donors, the United Way, and private foundations, the VNA also receives financial support from cities and towns across Chittenden and Grand Isle Counties. This funding helps to cover the cost of unreimbursed care provided by the VNA to residents of those cities and towns.

In September 2006, the VNA was awarded a multi-year federal grant from the "Promoting Responsible Fatherhood" demonstration program. Now in its third year, the VNA leads a collaborative program that includes the Winooski Family Center, the Milton Family Center, and the Family Center of Northwestern Vermont. The VNA works with these partners to replicate the Fathers and Children Together (FACT) program in their settings. The grant allows the VNA to strengthen the fatherhood focus of the VNA Family and Children's Services, to develop father-focused parent education classes, and disseminate information about the importance of father involvement throughout the community. Funding for this project was provided by the United States Department of Health and Human Services, Administration for Children and Families, Grant # 90FR002. These services are available to all eligible persons, regardless of race, gender, age, disability, or religion.

My baby is fussy and I have tried everything I know to calm him. How can the VNA help me?

As a new parent, Catherine King was experiencing the joys and the worries that come with having a baby. Her concerns grew, though, when her newborn, Lucas, seemed to have feeding issues, was not gaining weight, and had become so jaundiced that she was on the verge of bringing him to the hospital. At a friend's suggestion, Catherine contacted the VNA for help. VNA nurses help answer questions new parents might have, do post-partum check-ups on both the new mother and the baby and can even offer help and support with breastfeeding. The VNA Family and Children's Services program has a caring and skilled team of nurses who can help make the adjustments from hospital to home easier for parents.

Shortly after Catherine called the VNA, Lynne MethenyLieb, a Registered Nurse who specializes in lactation support, arrived at her home to help. With suggestions from Lynne and the team of Family and Children's Services' nurses, some adjustments to Catherine's diet, and regular home visits to give Lucas check ups, both mom and baby (and dad, too) are adjusting well to their new lives together.

Madison-Deane Initiative

On April 6, 2009, more than 500 people turned out to hear a presentation by Rachel Naomi Remen, MD, a pioneer of integrative medicine. Remen's presentation, Embracing Mystery: Recognizing Wonder in Our Daily Lives, was sponsored by the Madison-Deane Initiative with support from many generous community friends. Madison-Deane Initiative launched their latest project this fall, the Vermont Palliative and End-of-Life Care Resource Connections website. Designed for professionals and lay people alike, the comprehensive website offers information, resources and personal stories to help patients, their families and their caregivers as they face a serious illness. Can my mother receive Hospice Care while she is in the nursing home?

VNA nurse Betsy Lyons visits Dolores Wilson as needed each week to provide hospice care. Betsy focuses on symptom management, coordination of other services and education to the nursing home staff, and she offers support to Dolores' family. She also collaborates with the staff at the nursing home where Dolores resides to create a comprehensive, individualized plan of care. Each day, the VNA hospice team provides quality end-of-life care to patients in a variety of settings -- homes, assisted living facilities, Vermont Respite House, and increasingly, in area nursing homes. staff already provides and allows the patient to remain in a familiar setting. Hospice nurses coordinate an interdisciplinary team of licensed nursing assistants, social workers, and therapists to optimize the physical well being of residents. Hospice also emphasizes the importance of emotional and spiritual support. Along with trained Hospice volunteers who provide support during the dying process and the Noyana Singers, a volunteer group of singers providing songs of comfort at the bedside, Hospice offers bereavement support not only to a patient's family but also to the nursing home staff.
