John D. Oliverio President and Chief Executive Officer Wheaton Franciscan Healthcare John D. Oliverio is the President and Chief Executive Officer of Wheaton Franciscan Healthcare. He joined the organization in 1984 as Senior Vice President of Finance, became Executive Vice President in 1989, Chief Operating Officer in 1998, and was appointed President and CEO in 2000. John has led the organization's move to an integrated business model. He received a bachelor's degree in business administration from Loyola University in Chicago, and a Master of Management from the J.L. Kellogg Graduate School of Management at Northwestern University in Evanston, Illinois. A certified public accountant, Oliverio serves as a board member for more than 25 organizations and associations.

Susan Boland Senior Vice President and Chief Operating Officer Wheaton Franciscan Healthcare ­ All Saints Susan Boland, Senior Vice President and Chief Operating Officer joined WFH-All Saints in September 1995. Prior to joining WFH-All Saints, Susan was employed as the Associate Vice President for Nursing at Shadyside Hospital in Pittsburgh, PA. She received a bachelor's degree in Nursing from St. Xavier University and a master's degree from Rush University. Susan is a Board member of The Healthcare Network and the Racine Art Museum.

Brenda Bowers, Senior Vice President, Organizational Changes and Leadership Performance Wheaton Franciscan Healthcare Brenda J. Bowers, PhD, serves as the Vice President of Organization and Leadership Development for the Wheaton Franciscan System. In this role, she is responsible for systemwide development of leaders, orientation of all new associates, associate opinion survey process, 360-degree evaluations of leaders, diversity and cultural competence initiatives, e-learning efforts, clinical development, and library services. Brenda has more than 25 years of health care experience in a variety of capacities including functioning as a nurse, laboratory technologist, educator, consultant, and as a leader in health care operations and administration. She received her bachelor's degree from St. Martins College and Seminary, her graduate degree from University of Wisconsin-Platteville, and her doctoral degree from Walden University. She is actively involved with her family and community as a Christian leader, role model and advocate.

Kenneth R. Buser President, South Market, Southeast Wisconsin Kenneth R. Buser, has responsibility for WFH ­ All Saints in Racine. He joined WFH in April 1999. Prior to joining WFH-All Saints, Ken was employed as President and Chief Executive Officer of Union Memorial Hospital in Baltimore, Maryland. He received a bachelor's degree in modern languages from the University of Notre Dame and a master's of business administration degree with a concentration in hospital administration from the Graduate School of Business, University of Chicago. Ken is a board member for Sustainable Racine, Racine Area Economic Development Corporation, Wisconsin Health and Hospital Association, The Healthcare Advisory Board with Congressman Paul Ryan, and is a Fellow, American College of Healthcare Executives.

Richard J. Canter Senior Vice President Strategy and Corporate Affairs Wheaton Franciscan Healthcare Richard J. Canter, Senior Vice President Strategy & Corporate Affairs, is responsible for all legal services across WFH, including retention of outside legal counsel, corporate compliance and contract management. Richard brings 30 years of experience to this role. Most recently, he served as a partner and a member of Michael Best & Friedrich's Health Care Practice Group. Since 1976, he has provided legal representation to health care clients on regulatory and reimbursement matters, corporate and tax issues, medical staff matters, bioethics, Medicare-Medicaid anti-fraud and abuse, and issues affecting non-profit institutions. In addition, he has significant experience representing clients with respect to complex transactions, such as mergers and acquisitions, hospital affiliations and joint ventures among health care providers. Richard received a law degree from Hofstra University and a bachelor's degree from the University of Wisconsin-Madison. Stephen Cardamone, DO Senior Vice President & Chief Medical Officer Wheaton Franciscan Healthcare Stephen M. Cardamone, DO, is Senior Vice President and Chief Medical Officer of Wheaton Franciscan Healthcare. On a systemwide level, Dr. Cardamone has oversight for Quality and Patient Safety, the Electronic Health Record (with a focus on supporting clinical care delivery) and Care Management. Additionally, Dr. Cardamone is the operational executive leader for Wheaton Franciscan Cancer Care in Southeast Wisconsin and Rehabilitation Services in the North and Central Markets. Prior to his current position, Dr. Cardamone served as Senior Vice President and Chief Medical Officer for WFH ­ Iowa, in Waterloo where he was responsible for several functions including medical staff affairs, quality improvement, patient safety, case and risk management as well as operational areas of dialysis, cancer and home health. He also served in a clinical executive leadership and advisory role for Covenant Clinic. Dr. Cardamone also practiced at Covenant Clinic in Jessup for 10 years. He received his medical degree from Des Moines University, College of Osteopathic Medicine and Surgery and completed a Family Medicine residency at the University of Iowa, in Waterloo. Dr. Cardamone earned a

master's degree in Administrative Medicine and Population Health from the University of Wisconsin in Madison. Robert De Vita Senior Vice President and Chief Operating Officer Wheaton Franciscan Medical Group Bob De Vita is the Senior Vice President and Chief Operating Office for Wheaton Franciscan Medical Group. Bob has been in the healthcare field as a senior executive for more than 25 years and joined Wheaton Franciscan Healthcare in 2006. Prior to joining WFH, Bob served as Vice President for Resource Management at Ministry Health Care in Milwaukee and Chief Administrative Officer of Ministry Medical Group, and as Chief Administrator for Marshfield Clinic in Marshfield, Wisconsin. Bob received his bachelor's degree in Business Administration and his Master of Business Administration from University of Wisconsin-Milwaukee. He served as a National HMO Fellow at Georgetown University and at Marshfield Clinic. Additionally, Bob is a Certified Medical Practice Executive, Lean Six Sigma Project Champion and Green Belt, and serves as a member of the Advisory Board for the Sheldon B. Lubar School of Business at University of Wisconsin-Milwaukee.

Coreen Dicus-Johnson Senior Vice President, Payer Contracting and Revenue Operations Wheaton Franciscan Healthcare Coreen Dicus-Johnson is the Senior Vice President of Payer Contracting and Revenue Operations for WFH. She is responsible for the revenue operations for the Southeastern Wisconsin health care operations. Additionally, she negotiates and manages the payer contracts for the system's Southeastern Wisconsin, Illinois and Iowa health care ministry, which include 14 hospitals, four long-term care facilities, two home health agencies, more than 3,500 physicians, and 70 clinic sites with approximately 600 employed physicians. Before joining WFHC, Coreen was an employee benefits consultant at Frank F. Haack & Associates, Inc. She also spent approximately nine years at Blue Cross Blue Shield of Wisconsin in various roles as an attorney and most recently as a director of corporate development. She holds a law degree from DePaul University College of Law in Chicago and a bachelor's Degree from Marquette University in Milwaukee. Jack Dusenbery President and Chief Executive Officer Wheaton Franciscan Healthcare - Iowa Jack Dusenbery joined WFH-Iowa in Waterloo as President and Chief Executive Officer in June 2003. Prior to joining WFH-Iowa, Jack served as Chief Executive Officer of Tenet Healthcare's East Cooper Regional Medical Center in Charleston, South Carolina. Jack received a bachelor's of science and a Master of Arts from the University of Iowa. In addition, he is a diplomat in the American College of Healthcare Executives.

Wayne Frangesch Senior Vice President, Human Resources Wheaton Franciscan Healthcare Wayne Frangesch serves as Senior Vice President of Human Resources and Organization Development for Wheaton Franciscan Healthcare. Wayne joined the organization in June, 1996 when he started working as an HR Representative at St. Francis. He has worked in various HR leadership roles in Milwaukee, Racine, and Iowa, and was appointed to Vice President ­ Human Resources in January, 2006 and to his current role in July 2009. He originally began in the System in 1996 working in progressively larger Human Resources leadership roles in Milwaukee, Racine and Iowa. Wayne holds a bachelor's degree in Business Administration with concentration on Human Resources and Labor Economics and a master's degree in Economics from the University of Wisconsin-Milwaukee. During his tenure with WFH in Wisconsin, Wayne has also taught as a member of the adjunct faculty for Cardinal Stritch University and now serves as a mentor for graduates entering the workforce as part of UW-Milwaukee's Economics Mentoring Program. James Gresham President Wheaton Franciscan Continuing Care and Allied Services Jim Gresham is President of Continuing Care & Allied Services. He is responsible for Wheaton Franciscan sub-acute and transitional care facilities in the Milwaukee area, as well as Wheaton Franciscan Home Health & Hospice, Senior Health, Retail Pharmacy, Infusion Services, Congregate Living Pharmacies and home health medical products. Jim's tenure with the system extends to 1989, when he joined Wheaton Franciscan Healthcare as President of Marian Franciscan Center. Prior to that, Jim built an extensive history in continuing care, including leadership positions in the areas of transitional care and behavioral health throughout Wisconsin. He was involved in crafting Wisconsin state regulations on long-term care while employed with the Wisconsin Nursing Home and Ombudsman Program in the offices of Governors William Schreiber and Lee Dreyfus. A pioneer in the provision of sub-acute care in southeastern Wisconsin, Gresham has spoken and has been published nationally on topics relating to sub-acute care and continuing care services. He is professionally licensed as a Wisconsin Nursing Home Administrator, and holds a bachelor's degree in Economic/Political Science from UW-Whitewater and a master's in Political Science/Public Administration from UW-Milwaukee.

Sarah Herzog Senior Vice President and General Counsel Wheaton Franciscan Healthcare Sarah Herzog is the General Counsel for WFH. She is responsible for legal services across WFH system and heads up the Office of General Counsel. Attorney Sarah has provided legal representation to the WFH system since 1996. Before joining WFH in 2005 as in-house legal counsel, Sarah was an equity partner with the law firm of Michael Best & Friedrich where she served as the Chair of the firm's Health Law Practice Group. She holds a law degree from SUNY Buffalo and a post-graduate LL.M. in Health Law from Loyola University Chicago. She earned her undergraduate degree from SUNY Stony Brook. Sarah has built a reputation as one of the Midwest's leading attorneys specializing in health law, regularly speaking and teaching on health law related topics. In 2003 she was named as one of Chicago's "40 Under 40" Illinois Attorneys to Watch.

Roberta Johnson Vice President, Operations, Central Market, Southeast Wisconsin Wheaton Franciscan Healthcare Roberta Johnson, is Vice President ­ Operations for the Central Market in Southeast Wiscsonsin. She oversees clinical operations for WFH ­ Franklin and St. Francis. Roberta joined Wheaton Franciscan Healthcare in 1985 and served as Nursing Supervisor for St. Francis Home Health, Director of Nursing Administration at St. Francis, Vice President of Ambulatory Services at WFH ­ Wauwatosa, and most recently as Vice President of Business Development. She was appointed to her current role in September 2008 as part of a restructuring of the Southeast Wisconsin Region. She was instrumental in the programming, design and development of our Wauwatosa, Franklin, and Brown Deer sites. She also played a key role in on-boarding of Metro Physicians.

Dan Mattes President, Central Market, Southeast Wisconsin Wheaton Franciscan Healthcare Dan Mattes has responsibility for WFH ­ Franklin and St. Francis. He has been with WFH since 1992 serving in a series of progressive positions including financial analyst, strategic financial planning, cash management and treasury services, and most recently as Senior Vice President, Business Development, for WFH during which time he oversaw business development, physician relations, and new program and service development. Dan was promoted to his current position as president of the Central Market in September 2008 as part of a restructuring of the Southeast Wisconsin Region. He has a broad knowledge of our complex business environment evidenced by his significant contributions to the development of The Wisconsin Heart Hospital and WFH - Franklin.

Norma McCutcheon Senior Vice President, Operations, North Market, Southeast Wisconsin Wheaton Franciscan Healthcare Norma McCutcheon oversees clinical operations at WFH - Elmbrook Memorial, St. Joseph, The Wisconsin Heart Hospital, and Wauwatosa for joined the organization in 2001 and played a key role in the planning, construction and operational phases of The Wisconsin Heart Hospital in collaboration with cardiac, vascular and specialty physicians. Previously Norma served in various clinical and administrative roles with major hospital systems including Vice President and Chief Nursing Officer (CNO) with Hahnemann University Hospital in Philadelphia, Vice President of Critical Care Services at Mainline Health, and CNO of Lankenau Hospital, Wynnewood, Pennsylvania. Norma received a bachelor's degree in Nursing from Rockford College and an MBA from Northern Illinois University. She was named Senior Vice President, Operations, North Market in Sept. 2008 as part of a restructuring of the Southeast Wisconsin Region.

Loren M. Meyer, MD President and Chief Executive Officer Wheaton Franciscan Medical Group Loren Meyer, MD, is the President and Chief Executive Officer of Wheaton Franciscan Medical Group in Southeast Wisconsin. In this role, Dr. Meyer is responsible for the overall direction and management of the medical group including the development and implementation of strategic goals and action plans in coordination with the strategic planning process for the Wheaton Franciscan System. Dr. Meyer joined Wheaton Franciscan Healthcare in 1986 and has served in a variety of roles including Vice President of Quality and Medical Education and, most recently, Chief Quality Officer and Vice President of Medical Group Operations. Dr. Meyer received his medical degree from West Virginia University and completed his pediatric residency at Lutheran General Hospital in Park Ridge, Illinois.

Daniel E. Neufelder President and Chief Executive Officer Affinity Health System Daniel E. Neufelder joined Affinity Health System in 2006 as its president and chief executive officer. He has dedicated his career since 1979 to the health care industry. Under his leadership, Affinity has embarked on the LEAN Management journey and is focused on both growth initiatives and service excellence. Prior to joining Affinity, Dan served for 12 years as the executive vice president and chief operating officer of Memorial Hospital of South Bend, Ind. He has been active in professional organizations and has served in leadership capacities with the American College of Healthcare Executives and the Healthcare Financial Management Association, as well as the Oshkosh Chamber of Commerce Board of Directors and the UW Oshkosh Chancellor's Council of Advisors. Dan is the recipient of the 1997 Young Healthcare Executive of the Year Award for Northern Indiana. He received his bachelor's degree from the University of Southern Indiana and his master's degree in business administration from the University of Indianapolis.

Dave Olejniczak Senior Vice President, Operations, Iowa Wheaton Franciscan Healthcare Dave Olejniczak joined Wheaton Franciscan Healthcare - Iowa in January 2004 as regional Chief Operating Officer. Prior to joining Wheaton, Dave served in executive positions with Evanston Northwestern Healthcare in Chicago and ProMedica Health System in Toledo, Ohio. Dave is actively involved in the American College of Healthcare Executives as a Fellow and is a founding Board member of the Iowa Chapter. Dave received his Bachelor of Science in Industrial Engineering and Master's of Business Administration from the University of Toledo.

Terri Rocole Senior Vice President, Mission Services Wheaton Franciscan Healthcare Therese M. Rocole serves as the Senior Vice President of Mission Services for Wheaton Franciscan Healthcare. Terri has been employed by Wheaton Franciscan Healthcare since October, 1988, originally working in various administrative and leadership roles within Laboratory Services. In January, 2004, she was promoted to Director - Mission Services and later Regional Vice President - Mission Services at Wheaton Franciscan Healthcare ­ All Saints, in Racine. She was promoted to Vice President ­ Mission Services in January, 2007 and to her current role in July 2009. In her role, Terri is responsible for systemwide mission integration, spiritual care and ethics. She holds a bachelor's degree from the University of Wisconsin ­ Oshkosh, a master's degree in Business Administration from Athabasca University in Canada, and a Master of Education in Ministry degree from Cardinal Stritch University in Milwaukee.

Richard O. Schmidt, Jr. President and Chief Executive Officer United Hospital System Richard O. Schmidt, Jr., has served as President, Chief Executive Officer and General Counsel of United Hospital System since 1986. He was appointed to that role following two years of service as the Executive Vice President, Chief Operating Officer and General Counsel for United. Prior to joining United, Richard served as a health care attorney and consultant for Management Science Associates, Inc. Richard earned his bachelor's degree from the University of Arizona in Tucson, and his juris doctorate and post-doctorate degrees from the University of Missouri. He is a diplomat in the American College of Healthcare Executives; a member of the American Bar Association, the Wisconsin Bar Association and the Missouri Bar Association; and a member of numerous civic and community organizations.

Greg Smith Senior Vice President and Chief Information Officer Wheaton Franciscan Healthcare Greg Smith has served as senior vice president and chief information officer for WFH since 1999. In his role he is responsible for all aspects of planning, implementation and support information related technologies and services for Wisconsin, Illinois and Iowa. Since taking on this role, Greg has developed and begun implementation of a comprehensive clinical strategy for the System for the redesign of care delivery processes and deployment of an Electronic Health Record, and led many other initiatives. He began his career with WFH in 1984 serving in increasingly progressive roles. Greg also serves on the Strategic Advisory Council to McKesson Provider Technologies and the Healthcare Executive Committee of Hewlett Packard. Greg received his MBA from Cardinal Stritch University, a Bachelor of Business Administration from the University of Iowa and a Bachelor of Arts in Sociology from Monmouth College in Illinois. Greg is active in many community organizations and has lectured at Marquette University, the Academy of Health Services Marketing, the American College of Health Care Marketing, the American Academy of Orthopedic Surgery and several corporations. Jon W. Sohn Chief Financial Officer Wheaton Franciscan Healthcare Jon Sohn is Chief Financial Officer for Wheaton Franciscan Healthcare. Jon joined the former Covenant Healthcare in May 2003 as Regional Vice President, Financial Services. The Marquette University graduate previously worked as a Senior Manager at KPMG, LLP where he consulted with our organization as one of his major clients. Jon was appointed to Vice President of Financial Services in July 2007 in connection with the SE WI reorganization. He was named one of Milwaukee's Forty under 40 in 2008.

Debra K. Standridge President, North Market, Southeast Wisconsin Wheaton Franciscan Healthcare Debra Standridge has responsibility for WFH - Elmbrook Memorial, Wheaton Franciscan (St. Joseph Campus and The Wisconsin Heart Hospital Campus), Wauwatosa, and Brown Deer. She joined WFH in 2001 as Vice President ­ Operations for St. Francis and was appointed president in 2003. She was named President of the North Market in September 2008 as part of a restructuring of the Southeast Wisconsin Region. Previously, Debra served for seven years as Administrator for Cardiology, General Medicine, Geriatrics and Oncology at Hurley Medical Center in Flint, Michigan. She received a bachelor's degree in music therapy and music education from Michigan State University and a master's degree in Public Health from the University of Michigan ­ Ann Arbor. Debra holds strong leadership skills in exceeding operating margins, reducing expenses, and in general, creating a successful organization. Debra has demonstrated an extraordinary ability to engage and empower key stakeholders, whether they are direct-line managers, staff, political leaders, or donors.

Jon L. Wachs Senior Vice President and Chief Administrative Officer Wheaton Franciscan Healthcare Jon L. Wachs is the Senior Vice President and Chief Administrative Officer for Wheaton Franciscan Healthcare. Wachs has been working in health care for more than 30 years, including eight years with WFH in Milwaukee. Jon served as Executive Vice President in Milwaukee prior to taking on a corporate position in June 2000. Previously, Jon served as Chief Operating Officer for Santa Rosa Health System, San Antonio, Texas, and Chief Operating Officer for St. Mary's Hospital, Milwaukee. He also worked as a management consultant for a national consulting firm. Jon holds a bachelor's degree from St. Olaf College in Northfield, Minnesota, a master's degree from Marquette University in Milwaukee, and a master's of management degree from the Northwestern University Kellogg Graduate School in Evanston, Illinois. Nancy Weber Chief Nursing Officer, Iowa Wheaton Franciscan Healthcare Nancy joined WFH-Iowa in 1972 as a staff nurse in the ICU and has since held sever positions including Critical Care Educator, House Supervisor, Director of Education, CQI Advisor. She was promoted to CNO for the Iowa Region in 1997. Prior to joining Wheaton Franciscan Healthcare, Nancy worked as a staff nurse in critical care at St. Mary's Hospital in Rochester Minnesota. She received her diploma in nursing from St. Mary's Hospital, her BSN from Upper Iowa University, and her MSN from University of Iowa. Nancy was selected in 2002 as Wharton Nurse Executive Fellow. She is also a member of AONE and IONL.

Kathleen C. Yosko President and Chief Executive Officer Marianjoy, Inc. Since 1998, Kathleen Yosko has served as President and Chief Executive Officer of Marianjoy, Inc., Marianjoy Rehabilitation Hospital and Marianjoy Medical Group in Wheaton, Il. Prior to joining Marianjoy, Kathleen served as President and Chief Executive Officer of Schwab Rehabilitation Hospital and Care Network, a 125-bed comprehensive rehabilitation facility. She began her career as a registered nurse/clinical specialist in rehabilitation at the Veterans Hospital University and Highland Drive Campuses in Pittsburgh, Pennsylvania during the Vietnam War. Kathleen is currently serving as chair of the American Medical Rehabilitation Providers Association (AMRPA), a nonprofit trade organization representing freestanding rehabilitation hospitals, rehabilitation units in general hospitals, outpatient rehabilitation facilities, several skilled nursing facilities, and home health agencies that provide medical rehabilitation services to over 700,000 people annually. She is also active in many other professional organizations.
